Responding to this survey is completely voluntary and confidential. Information obtained is used for identifying areas of service we provide which may need improvement. All results of the survey you provide will be compiled by in-house quality control personnel and used only as statistical information. You can either submit the form electronically (choose the "Print Form" button on the top) or print the completed form and mail it to Station 81.
We at the South Ogden City Fire Department are always looking at ways to improve the services we provide you, our customers. One area of information we consider invaluable is comments provided by the community we serve. We have developed a Fire & Medical Services Survey to assist us in gathering input that will help identify our “good” areas of service as well as services we need to improve or implement.
